CPR/AED & First Aid Training for FP&M Employees

medical cross symbolThis CPR/AED and First Aid training is a 4.5-hour session that teaches ways to recognize and respond appropriately to cardiac and breathing emergencies. Participants will also learn ways to give immediate care to a suddenly injured or ill adult until more advanced medical personnel become available.

Successfully complete the course to receive a valid two-year Adult First Aid/CPR/AED certification. Space is limited and will be on a first-come first-served basis. Facilities Planning & Management and the Division of Recreational Sports bring you these CPR/AED class offerings, which are available on several dates in June, below:

To register, please complete a training request form (see below.)


How To Register

  1. Complete Form: Complete the FP&M Training Request Form for your department
    1. JUSTIFICATION FOR PROFESSIONAL DEVELOPMENT section: Enter your name, title, email address, phone number, and justification (reasons why the training relates to job)
    2. Check the checkbox “COURSE (2184)
    3. Event Title: Add CPR/AED and First Aid
    4. Event Date: Choose one of the available dates listed above and enter the corresponding event time in “Time” field
    5. Check the checkbox for “CERTIFICATION” as the event type
    6. Event Address: 797 W. Dayton Street, Madison, WI, 53706
    7. Check the appropriate checkbox for either “RENEWAL” or “NEW”
      1. Those renewing their certification should enter the date that that their past certification will expire under “Expires on”
      2. Others can select “NEW” and leave “Expires on” empty
    8. EXPENSES AND FUNDING section: Enter $75 as Registration/Fees
    9. Leave the rest blank
  2. Submit for Approval: Submit the completed form to your supervisor for review and approval.
  3. Submit to FP&M Training: After approval, you or your supervisor should scan/email the signed form to FP&M Training at training@fpm.wisc.edu
  4. Confirmation: you will receive a registration confirmation email to let you know that your form has been approved and you are registered for the class.
